A prior INCOSE paper addressed several famous failures from the viewpoint of the impact of requirements, verification, and validation. That paper has been used for pre‐reading material and initial class discussion in a course on system integration that also includes verification and validation. The discussion also notes that the original paper does not address the impact of integration on these failures. This paper adds that viewpoint and includes several other instances to develop several aspects that should be considered in systems integration.
