小腸大量切除を行った16例および成犬における実験的検討から術後の病態生理を検討し, 術後管理の重要性について述べた.次の結論をえた.1) 16例中残存小腸の最長は70cm, 最短5cmであったが, 下痢回数, 便中脂肪排出量には個人差があり, 残存小腸の長さ, 回盲弁の有無との関連はなかった.2) おのおの症例の経過にふさわしい術後の栄養対策が必要 (電解質補正, ビタミン剤, 脂肪乳剤等の投与) である.3) PGE2量が減少し, 胃の生体防御が低下しているので潰瘍の予防が必要である.4) 耐糖能異常を認めた例や, 膵組織中必須脂肪酸の低下などがみられ, 膵炎の発生予防にも注意すべきである.5) 逆蠕動腸管の挿入は非挿入例に比較して有意差は認めなかったが, 栄養吸収の改善に有効な例があった.
