我々は前回,整形外科と災害外科 第66巻 第2号にて,上腕骨近位端骨折後の偽関節に対するMODE Proximal Humeral Plate (以下 PHP)の有用性を報告した.PHPは,近位に9~10本の screwが挿入で き,腋窩神経より遠位側から骨頭頚部内側への infero-medial screw挿入が特徴で,強固な初期固定性が 得られる.今回,上腕骨近位端骨折の初回使用例も含めて報告する.【対象・方法】2015年4月~2016年 12月,術後6ヶ月以上経過観察できた14例(初回手術12例,偽関節に対する手術2例),男性4例,女 性8例,平均年齢71歳を対象とし,単純 X線評価(骨癒合の有無,矯正損失),骨頭壊死も含む術後合 併症,最終診察時の日本整形外科学会肩関関節疾患治療成績判定基準(JOA score)と肩関節 ROMにつ いて調査した.【結果】13例に骨癒合を認め矯正損失は認めなかった.1例に骨頭壊死を認めた.JOA scoreは平均88点であった.PHPは矯正損失がなく強固な固定が可能な上腕骨近位端骨折に有用なイン プラントである.
