肺癌 根治 手術例 に対 し,術 中 よ りシベ レス タ ッ トナ トリウム を投 与 して,良 好 な結果 を得 た。74歳,男 性。脊 椎 管狭 窄症 に て整形 外科 入院加 療 中,胸 部 異常 影 を指摘 され,同 年6月 当科 に紹 介,転 科 とな る。精査 に て左肺 癌 と診断,肺 気腫症 に よる低 肺機 能 のため 術前 呼吸 訓練 を3週 間行 ってか ら手術 施行 した。左 肺上 葉切 除 した と こ ろ,微 小血 栓塞 栓症 に よ る と思 われ る残存左 肺下 葉 の鬱 血が進 行 し,左 肺 全摘 とな った。手術拡 大 の過大 侵襲 か らSIRS:systemic inframmatory response syndromeに よ る肝 障害 の可能 性を考 え,術 中 よ りシベ レス タ ッ トナ トリウム水 和物(以 下 エ ラス ポー ルR)投 与 開始 した。 経過 中エ ラス ポー ルRは0.2g/kg/hrで 術 後14日 間使 用 した。術後 呼 吸状 態は不 良 で,人 工 呼 吸管理 を要 したが,徐 々 に 回復 して術後7日 目に抜管 し,術 後第25病 日 退 院 した。
