症 例 は18歳 の 男性。 主訴 は呼 吸 困難。既往 歴 に脳性 麻 痺 に よる精神 遅滞 とてんか ん発 作が あ る。1999年 頃 か ら 頸部 腫 脹の 進行 と ともに呼吸 困難 とてんか ん発作 の 増加 を認 め る よ うに な り来 院 した。 前頸部 に弾性 軟 ・表 面平 滑 ・可動性 不 良 な腫 瘤 を触知 した。 頸部CTで 甲状 腺右 葉 は54×37×113mm,左 葉 は65×54×80mm大 に腫大 し, 気 管 は胸 骨切 痕 を中心 に長径24mm,横 径4mmの 狭 窄 を認 め た。甲状 腺腫 に よる気管 狭窄 と診 断 し,気 管支鏡 ガ イ ド下 に挿 管後,甲 状 腺全 摘術 を施 行 した。 甲状 腺 は,前 頸 筋 ・気管 ・食道 等へ の浸 潤 は認め ず,圧 排 性 の発 育 を示 し,病 理 組 織診 断 は腺腫様 甲状 腺腫 であ った。 術後24時 間の 挿管 ・人工 呼吸 器管 理 を行 い,合 併症 な く第8 病 日に退院 した。術 後3年 間,呼 吸 困難 とて んか ん重症 発作 は認 め てい ない。
