症例 は59歳,男 性 。1週 間 前か ら咳嗽 が続 い てい たが,激 しい咳嗽 の直後,突 然 の心窩 部痛 と冷 汗が 出現 し近 医 を受診 。腹部CT検 査 で後 腹膜 血腫 が疑 われ 当科紹 介 となる。来院 時,軽 度の 貧血 を認 め る もの の意識 は清 明, 上腹 部 に径50mm大 の圧 痛 を伴 う腫瘤 を触 知 した。造 影CT検 査 で上 腸 間膜動 脈根 部 周囲 を中心 に後 腹膜 腔 に血腫 が広 が って お り,十 二指 腸 お よび膵臓 が腹 側 に圧 排 され て いた。膵頭 部領 域 の動脈 瘤破 裂 を疑 い,直 ちに血 管造 影検査 を行 っ た。上腸 間 膜動 脈か らの造 影 で後上 膵十 二指 腸動 脈 に動脈 瘤が み られ たため,同 部位 か らの 出血 と 考 え経 カ テー テル動脈 塞栓 術(TAE)を 施行 し止血 した。後腹 膜 血腫 の圧迫 に よ る と思 われ る十二 指腸 の通過 障 害 が約3週 間み られ たが,血 腫 の吸収 に伴 って軽快 した。
