症 例 は37歳,男 性。高 尿酸 血症 経過 観察 中,LDH上 昇 を認め 腹部 精査 施行 。肝 左葉 に径10cm大 の腫瘤 病変 を指 摘 され た。 入院 時血液検 査 で は,肝 炎virus,腫 瘍marker(AFP,PIVKAII)を 含 め異 常 な く,腹 部computed tomographyで は動脈相 で強濃染 す る辺縁 不整 な結 節 を認 めた。 造影 超音 波検 査 では流 入動 脈 の発 達 と腫 瘍 の 濃 染,周 囲組織 と同等度 の造 影剤 の停 滞 を認め た。Magnetic resonance imaging with contrast medium of superparamagunetic iron oxideで は ク ッパー 細胞 の少 な い腫 瘤 が予 想 され た。 肝腫 瘤生検 では異 型の 少 ない肝細 胞 と 胆 管 を含 む 門脈域 と線維性 結合 織 を認め,限 局性 結節過 形 成(FNH)が 疑 われ たが,必 ず しも腫瘍 性病 変 を否定 で きず,肝 左 葉切 除施行 した。切 除標 本 には 中心性瘢 痕 は なか ったが,組 織 学 的に典 型的FNHで あっ た。診断 困 難例 でかつ安 全 に肝切 除可 能 な場合,切 除 に よ り組織 診 断 と治療 を施行 す る事 は妥 当な治療 方針 と考 え る。
