症 例 は77歳,女 性 。平 成8年 か ら糖 尿病性 腎症 にて血 液透析 中で あっ た。平成10年7月 か ら肝細 胞癌(以 下HCC) の診 断に て,過 去2回 保 存的 治療 と して,PEITとRFA(経 皮 的 ラジ オ波 熱凝 固療 法)を 施行 され た。PIVKA-II を指標 と して,腫 瘍 の増 大 お よび治療効 果 を評価 して いた。平 成12年5月 再 びPIVKA-IIが 上 昇(12,522mAU/ ml)し 精査 の 結果,S3に20mmの 腫 瘍 と3個 の肝 門部 の リンパ 節腫 脹 を認め た。HCCの 再発 と リンパ 節転 移 の診 断で,同 年8月10日 肝 外側 区切 除術,肝 門部 リンパ 節郭 清 を施 行 した。術 後13病 日でPIVKA-IIは(468mAU/ml) まで低 下 した。術 後合 併症 な く経過 してい たが,患 者 は第23病 日脳 幹部 梗塞 の ため 死亡 した。病 理組 織検 査 では リンパ 節 はいず れ も原発 巣 と同 じHCCの 転移 であ った。
