1.強度変調放射線治療(IMRT)と 従来の放射線治療との違い 強度変調放射線治療(IMRT)とは,1988年に Brahme 博士らが考案した照射技術である.定義としては「逆 方向治療計画(インバースプラン)に基づき,空間的, 時間的に不均一な放射線強度を持つ照射ビームを多方向 から照射することにより,病巣部に最適な線量分布を得 る放射線治療法」となる.詳しく述べると従来の3次 元原体照射のような均一な放射線強度の照射とは異な り,リニアックの照射口付近に取り付けられている multi―leaf collimater (MLC)(図1)を照射の際中に動 かすことにより時間的・空間的に不均一な放射線強度を 持つ照射野を複数の方向から作成し,これを合算する ことで任意の線量分布図を描くことが可能となった (図2).しかし,どのように強度変化をつけるのが適切 かを人間の頭で考えることは極めて困難であるため, 頭頸部癌に対する強度変調放射線治療 (IMRT : Intensity―modulated radiotherapy)の歴史と今後の展望
