Реферат Мета. Оцінити зміни якості життя хворих з ішемічною кардіоміопатією після хірургічної реваскуляризації та трансплантації стовбурових клітин пуповинної крові. Матеріали і методи. Проведено аналіз змін якості життя 113 хворих з кардіоміопатією ішемічного генезу. У залежності від методу лікування пацієнти були розподілені на такі групи: 1–ша – 38 (33,6%) пацієнтів, яким було виконано коронарне шунтування, 2–га – 35 (31,0%) пацієнтів, яким було виконано стентування коронарних артерій, 3–тя – 20 (17,7%) пацієнтів, яким на доповнення до хірургічної реваскуляризації була виконана процедура трансплантації стовбурових клітин пуповинної крові; 4–та – 20 (17,7%) пацієнтів, яким була призначена тільки консервативна терапія. Результати. При оцінці ефективності застосування стовбурових клітин пуповинної крові за критерієм зменшення кількості балів за опитувальником The Minnesota Living with Heart Failure Questionnaire (MLHFQ) на 50,0% через 12 міс спостереження приріст терапевтичної ефективності відносно покращення якості життя після комплексного застосування реваскуляризації міокарда та трансплантації стовбурових клітин пуповинної крові становив 75,0% порівняно з консервативною терапією, 53,7% порівняно з коронарним шунтуванням та 48,6% порівняно зі стентуванням коронарних артерій. Доведено суттєве підвищення «шансів» досягнення даного ефекту, а також вірогідне кумулятивне відношення шансів – 4,85 [2,44 – 9,64]. За даними опитувальника SF–36 отримано вірогідно більш високі показники як фізичного, так і психічного компонентів здоров’я у хворих, яким реваскуляризацію міокарда доповнювали трансплантацією стовбурових клітин пуповинної крові. Висновки. Після трансплантації стовбурових клітин пуповинної крови, якою доповнюють реваскуляризацію міокарда у хворих з ішемічною кардіоміопатією, зменшуються прояви серцевої недостатності, частково відновлюються толерантність до будь–яких видів фізичного навантаження, включаючи повсякденну роботу й самообслуговування, а також покращуються показники фізичного та психічного компонентів здоров’я. Ключові слова: кардіоміопатія; серцева недостатність; стовбурові клітини пуповинної крові; якість життя. Abstract Objective. To estimate the changes of the patients’ quality of life in those, suffering ischemic cardiomyopathy after surgical revascularization and transplantation of the stem cells, obtained from umbilical blood. Materials and methods. There was analysis conducted on changes in quality of life in 113 patients, suffering cardiomyopathy of ischemic genesis. In accordance to the therapeutic method applied, the patients were divided into four groups: first – 38 (33.6%) patients, in whom coronary shunting was conducted, the second – 35 (31.0%) patients, in whom coronary arteries stenting was done, the third – 20 (17.7%) patients, in whom surgical revascularization was added by transplantation of the umbilical blood stem cells; and the fourth – 20 (17.7%) patients, in whom conservative therapy only was administered. Results. Estimation of efficacy of the stem cells, obtained from umbilical blood, was conducted, in accordance to the points quantity reduction, using the data from The Minnesota Living with Heart Failure Questionnaire (MLHFQ), by 50.0% in 12 mo of observation. This enhancement of therapeutic efficacy in aspect of the quality of life improvement after complex application of the myocardium revascularization and transplantation of the umbilical blood stem cells have constituted 75.0%, comparing with conservative therapy, 53.7% – comparing with coronary shunting and 48.6% –comparing with the coronary arteries stenting. There was proved the essential raising of probability for achievement of this effect, as well as a probable cumulative restoration of the chances – 4.85 [2.44 – 964]. In accordance to the SF–36 questionnaire answers data; trustworthily higher indices for the physical and psychological health components were obtained in patients, in whom myocardial revascularization were added by the umbilical blood stem cells. Conclusion. After transplantation of stem cells, obtained from umbilical blood, as the additive to the myocardial revascularization for treatment of ischemic cardiomyopathy, the signs of cardiac insufficiency have diminished, tolerance for any physical loading partly renewed, including solemn round of industry and a self–servicing, as well as the indices for the physical and psychological health improved.
