. Оpen fractures of the shin bones with significant injury of soft tissues are belonging to the degree III traumatic affections (in accordance to Gustilho – Аndersen classification) and constitute 45.8 – 64% rate in structure of open fractures. Маterials and methods. Results of treatment in the clinic of 76 injured persons with open fractures of the shin bones in 2008 – 2015 yrs were analyzed. In patients of group І the external fixation method was changed to internal fixation, and in group ІІ – a definite treatment of fracture was conducted, using the external fixation apparatus without a change. Results. While estimating the results of surgical treatment of open fractures of the shin bones, using scale of Neer–Grantham–Shelton, in group І good results were noted in 27 (57.45%) patients, and in group ІІ – in 9 (31.03%). Conclusion. Treatment of the injured persons for high–energy trauma of the lower extremities constitutes a durable process and is accompanied by high rate of poor results. The main criteria for change of the fixation method from external one to internal are: uncomplicated healing of the wound, as well as absence of laboratory and clinical signs of inflammation.
