Objective: Duodenopancreatectomy is one of the most complex and challenging procedures, the laparoscopic approach is an alternative to take into consideration. The objective is to present the initial experience of a patient operated with Fully laparoscopic duodenopancreatectomy, as well as the technical aspects of the approach. Methods: Prospective case study. Patient characteristics, diagnosis, surgical technique, operative time, intraoperative and postoperative complications, hospitalization time, evolution and applicability of the approach were analyzed. Results: We report the procedure of a patient whose final diagnosis was cholangiocarcinoma, during the dissection the most laborious area was the hepatic pedicle, the retropancreatic tunnel and the uncinated process, the reconstruction was done by 3 anastomoses: telescoping pancreaticojejunal, termino-lateral hepatojejunal and termino-lateral gastrojejunal.
