要 旨:症 例 は47歳 の 男性.4,36,37,39歳 時 にお いて4回 の肺 炎 に罹 患 し,さ らに,39歳 時 に初 め て右 上肺 野 に気腫 性嚢 胞 を指摘 されて い る.昭 和63年9月 に気腫 性嚢 胞 と反対 側 に肺 炎 を発 症 し,抗 生剤 投 与に て軽快 したが,そ の3カ 月後,発 熱 と胸 部X線 写真上 異常 陰 影 が 出現 し,そ の精 査加 療 目的 で入院 とな った.胸 部X線 上,気 腫 性嚢 胞 は8年 前 と比 較 して,多 発 性 に増 大 してお り,更 に嚢 胞に接 して径4.6cmの 腫 瘤 陰影 を認め た.腫 瘤陰 影 は経皮 的肺 生検 に よ って低 分化 型腺 癌 と診 断 し,平 成1年3月 当院胸 部外 科 に て右 肺 上葉切 除術 を施行 した.上 葉肺 前面 に 巨大 な腫 瘤 と背面 か ら肺尖 部 にか けて気腫 性嚢 胞 が あ り,そ れ らには若 干 の索状癒 着 が あっ た.術 後8カ 月 目に非 定 型抗酸 菌症 が生 じ, 某 院に転 院 とな っ た. 〔肺癌33(6): 959~964, 1993〕
