The surgical anatomy of common atrioventricular orifice (CAVO) with tetralogy of Fallot (Tet), double outlet right ventricle (DORV) and complete transposition (CT) is not well delineated in the literature. Of 507 specimens of CAVO, 30 had Tet, 34 DORV and 17 CT. In Tet, the CAVO was of the complete type in 93%, with a free-floating anterior bridging leaflet, and in 7% it was of the intermediate type. In 21% the left ventricle was smaller than normal. The defect was subaortic in all hearts with CAVO. In DORV, the defect was subaortic, related to both vessels, related to neither, or subpulmonic. The great vessels were normally related, side by side, anteroposterior or, rarely, inverted. The same percentage of complete and intermediate forms of CAVO was present in DORV as in Tet. In 41% the left ventricle was smaller than normal, with deficiency of atrioventricular valvular tissue on the left side. In CT the defect was subaortic or unrelated to either vessel. The CAVO was complete, and the anterior bridging leaflet free-floating in all hearts except one that was of intermediate type. We believe these anatomic details are of surgical significance in these entities.
