Age is the main risk factor for cardiovascular disease.\udAge increases cardiovascular risk not only because it increases\udthe exposure time to, the number of, and severity of\udother cardiovascular risk factors, but it is also able to change\udthe system through a CV aging process. Aging per se causes\udan alteration of the vascular system, which involves the endothelial\udfunction, the vascular wall, and the related signaling\udcascades. These changes provide an important contribution to\udcardiovascular risk in the elderly population. At the same time\udlife expectancy is constantly increasing and the elderly deserve\uda good prevention strategy. New algorithms to predict\udcardiovascular events are under construction and validation.\udThese new algorithms are trying to integrate new biomarkers\udwith the most important cardiovascular risk factors, which\udare powerful predictors in adults. In our opinion, at the current\udstate of knowledge, treatment should be designed taking into\udaccount the characteristics of the patient, without making exceptions\udon the criteria of age. Also the goals of treatment\udshould be similar to those of young adult subjects if the elderly\udpatient has a good life expectancy, while the goals of\udtreatment in the frail elderly patient with co-morbidity and reduced\udlife expectancy should be to avoid complications of\udtreatment with less stringent targets
