In 2016, the German Human Biomonitoring Commission (HBM-C) published a statement on its decision to derive HBM-I values for Perfluorooctanoic acid (PFOA) and Perfluorooctanesulfonic acid (PFOS)*. The objective of this manuscript is to describe the methods and the prinicipal studies, on which this derivation is based. A systematic literature search for epidemiological and toxicological data on PFOA and PFOS has been undertaken. The following objectives have been pursued: (a) to identify health-related endpoints associated with internal exposure to PFOA or PFOS (blood concentrations), (b) to select those studies, which reported significant associations between internal PFAS dose and effect size/ rate, (c) to derive epidemiological points of departure from these studies. All studies were evaluated with respect to study design, quality of information, analytical and statistical methods and control for bias and confounding. Evidence for associations between PFOAand PFOS-body burden and health outcomes was seen for fertility and pregnancy, weights of newborns at birth, lipid metabolism, immunity, sex hormones and age at puberty/ menarche, thyroid hormones, onset of menopause as well as uric acid metabolism. Significant contrasts were reported for human blood plasma concentrations between 1 and 10 ng PFOA/ml, and 1–15 ng PFOS/ml, respectively. Within the reported ranges, the HBM-C has decided to set the HBM-I-values at 2 ng PFOA/ ml and 5 ng PFOS/ml blood plasma. The pathomechanisms involved do currently not seem to be clarified sufficiently to provide an explanation of the effects observed. Consistency of toxicological and epidemiological data has been considered. The data available do not appear to show proof of a genotoxicity of PFOA and PFOS. *Bundesgesundheitsbl 2016, 59:1364
